NEW PATIENTS:
Select your appointment time using our online scheduler. Please include the following in the appointment request form: (1) insurance plan name (2) insurance member ID (3) DOB (date of birth) (4) reason for the visit (5) how you heard about us (if a referral was sent to us, please include the date).
Once your appointment is confirmed, you will receive an email invitation that will allow you to check in online prior to your visit using our Patient Portal. This is an essential (and green) tool to communicate with our office.
